The medicine that the characters are discussing about is traditional Chinese medicine (though the story is not quite set in China per se). If you have ever had the unfortunate luck to try TCM (especially in liquid form), you will know that it can be extremely bitter, unpleasant, and hard to swallow. However the consensus is that the more bitter the medicine the more effective it is. Be thankful if you never have to try it!
